Required Registration Documents

The South Burlington School District requires the following documents as part of the registration process:
- Birth Verification with Proof of Age
- Health Documents
- Proof of Residence
The district MUST have all documents on file for your student to attend school. Families may:
- Choose to upload them during the online registration process. (RECOMMENDED)
- Provide them to the school or the district's Central Office.

Birth Verification with Proof of Age
- Certified copy of the student's Birth Certificate
- Valid, unexpired passport

Proof of Residency (South Burlington and Georgia Residents)
You are required to provide documentation of the student's residence within the service area of the school you wish to enroll in. Please use this South Burlington School District School Boundary Map for assistance by clicking the search icon (magnifying glass) and entering your street address.
ALL DOCUMENTATION SHOULD INCLUDE THE NAME OF THE LEGAL PARENT/GUARDIAN AND A VALID SBSD SERVICING AREA ADDRESS.
Please provide ONE of the following:
- Current mortgage papers or title
- Current property tax bill
- Renters agreement or copy of lease showing a valid SBSD Serving Area Address and name of the lessee (parent/legal guardian)
- If you cannot provide one of the above because you live with someone at a valid SBSD Servicing Area Address, please provide a notarized letter from that person which states you are living with them at that address. This letter must include the name of the parent/legal guardian, the name and address of the person you are living with in the SBSD Servicing Area, along with their proof of residency.
And
Please provide TWO of the following documents dated within the last 60 days with name and address indicated:
- Valid Vermont driver’s license
- Current utility bill in your name
- Valid Vermont identification card
- Valid automobile insurance card with the address showing
- Valid voter registration card or receipt
- Bank statement for last or current month (financial information redacted)
- Valid Vermont vehicle registration card

Proof of Residence (Grand Isle Supervisory Union Residents)
Students Attending SBHS or FHTMS from the Grand Isle Supervisory Union:
- The South Burlington School District will request/receive approved Tuition Vouchers directly from the Grand Isle Supervisory Union as Proof of Residency.

Health Documents (In-House PreKindergarten to Grade 12 Only)*
- Immunization Record (View the list of immunizations required by the VT Department of Health )
- Well-Care/Health Examination Form (required for prekindergarten students at Chamberlin Preschool, Orchard Preschool, and RMCS Preschool)
* Health Documents are NOT required for Act 166 registration.
